Jo Soh Bee Ching, the People's Association constituency director at Changi-Simei constituency, was fined $5000 for giving false information to the CPIB.
Ms Soh, 40, had told the CPIB that two statements signed by Chia Chee Sin had been signed in her presence on March 1 2009 and October 10 2010.
In fact, the forms were not sign in her presence and she had lied to CPIB officers about this fact.
The two forms were statements on conflict of interest disclosure that the CPIB required as Mr Chia was the youth executive committee chairman at Changi Simei CC during the time that he was also working for a company called J Force Events, which was dealing with the community club to organise an event.
By saying that the two statements were signed in her presence when in fact they were not, Ms Soh committed the offence of providing false information to a public servant.
Ms Ho has since been suspended from her duties at the CC.
The offence carries a maximum sentence of 1 year in jail, a fine up to $5000 or both.
