According to condom instructions, you should pull out BEFORE your penis becomes soft, that means, when your penis is erect and hard.

When your penis has already gone soft and small, you are withdrawing/slipping out of the condom leaving the condom.

I cannot remember ever having any condom slippage in the vagina, but I have had several condom breaking/tearing incidents.
