Time nuke 90% of US cities, make all coastal and border areas out of bound by radiation and too deadly to attempt fleeing. Leave only some central zone lake Nebraska Kansas still can take refuge, let the remaining small amount of survivors to suffer trapped there, badly cut off from the rest of the world. They can live in caves.
